Registration

New Student Registration Information:

The following documentation is required in order to register new students

1. Birth Certificate
2. Immunization Records
3. Photo ID of parent/guardian

4. Proof of Residency (one required)
        copy of rental lease
        most recent property tax bill
        contract of purchase (new home)

5. Additional Proof of Residency (one required)
        electric bill
        water bill
        home phone bill (not cell)
        car registration (not license)
        pay stub
        cable bill
        local/state/federal correspondence (such as..)
              SS
             Medicaid
             DSS
             IRS
             Food Stamps
             Court Documents

6. Affidavit of a Student's Domicile must be completed and notarized
